Details

This issue generally occurs for one of those reasons:

  1. The initiation or association form linked with your workflow is stored in a document library.

  2. The SharePoint server has configuration issues (corrupted workflow, insufficient disk space, etc.).

Solution

If your initiation or association form is stored in a document library, you can resolve this issue by migrating the document library into your destination site. Upon re-executing your workflows, the correct forms should be displayed.

If the form does not reside in a document library, it means that some configuration issues prevented it from being copied. You can first validate that the destination server has sufficient disk space. Also, you can make sure that the workflow at your source is not corrupted by republishing the workflow. If you have trouble identifying why the form is not being copied, don't hesitate to contact our technical support team.
